0000029: Discovered entities duplicate known ones

Description

If you manually add a place, Joe's Coffee for example, and Yelp discovery finds Joe's Coffee as well, both entries appear in the list separately, when they should actually be merged since they represent the same entity. There is a similar problem with people, for example you manually add a contact with a Twitter name, and Twitter discovery finds that same name as a local twitterer.
